HealthPro Mental Health Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote hospice care team in Miami, Florida. You will work closely with a diverse team of licensed mental health professionals, including psychiatrists and nurses, to jointly address the mental health needs of patients in our hospice program. • Conduct comprehensive mental health assessments for patients, considering their personal histories, current status, and unique psychosocial factors.
• Develop and implement individualized treatment plans that specifically target mental health concerns and enhance overall emotional well-being.
• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and trauma-informed care to address various mental health challenges.
• Facilitate both individual and group therapy sessions aimed at promoting coping skills, emotional regulation, and connection within the hospice community.
• Coordinate care with other healthcare providers, ensuring a comprehensive approach to mental health and continuity of care.
• Provide emergency mental health interventions and ongoing crisis support to patients and families as needed.

You will primarily work with patients receiving hospice care, many of whom face significant emotional and mental health challenges related to terminal illness. Your expertise will be crucial in helping patients navigate their feelings of anxiety, depression, and grief, thereby enhancing their quality of life during their end-of-life journey. Additionally, you will support families, helping them cope with emotional distress and facilitating important conversations surrounding care and loss.

• A Master’s Degree in Social Work (MSW) from an accredited institution.
• Current and valid LCSW license in the state of Florida.
• A minimum of two years of post-licensure experience, especially in a mental health or hospice setting.
• In-depth knowledge of mental health disorders and evidence-based treatments.
• Experience conducting comprehensive mental health assessments and developing appropriate treatment plans.
• Strong skills in crisis intervention and the ability to function effectively in high-pressure environments.
• Proficient in using electronic health records (EHR) systems for documentation and data management.

• Prior experience in hospice or palliative care settings is preferred.
• Familiarity with trauma-informed care practices and methodologies.
• Demonstrated ability to work within a multidisciplinary team focused on holistic patient care.
