Key Responsibilities- Provide direct clinical services to clients experiencing major mental illnesses or severe emotional disorders, both through virtual consultations and telehealth services.
- Conduct thorough assessments and develop individualized care plans tailored to the unique needs of hospice clients, coordinating with other healthcare providers, family members, and community resources.
- Engage in regular communication with clients and their families, offering emotional support and guidance through the stages of grief and bereavement.
- Monitor and evaluate client progress, making necessary adjustments to treatment plans to ensure therapeutic goals are met.
- Participate in interdisciplinary team meetings to discuss patient care and improve service delivery.
- Document all interactions and interventions in adherence to ethical and legal standards, ensuring timely completion of reports and data submission.
- Provide crisis intervention as needed, ensuring the safety and well-being of clients.
- Assume on-call responsibilities as assigned, providing crisis support during and outside program hours.
- Contribute to quality improvement initiatives and actively participate in agency committees focused on enhancing service delivery.

Qualifications- Current licensure as a Clinical Social Worker (LCSW) in Florida.
- A Bachelor’s Degree in Social Work, Psychology, or a related field; a Master’s Degree is preferred.
- A minimum of 2 years of experience in a clinical social work setting, ideally with a focus on hospice or end-of-life care.
- Strong knowledge of community resources, healthcare regulations, and ethical standards in social work.
- Experience working with healthcare teams and interdisciplinary groups.
- Familiarity with telehealth technologies and evidence-based practices in mental health.
- Prior experience in hospice care or with clients experiencing chronic illnesses.
- Skills in cultural competence and the ability to communicate effectively with a diverse population.
The salary range for this position is between USD 70,000 and 85,000 per year, commensurate with the level of experience and qualifications.
