Work Environment

Key Responsibilities
  • Conduct comprehensive psychosocial assessments for patients and their families at the time of admission, annually, and as clinically indicated. Utilize evidence-based social work theories and techniques to evaluate and address their emotional, social, and physical needs.
  • Produce and maintain electronic medical records as per departmental and healthcare regulations, preparing written reports and participating in hand-off communications to ensure continuity of care.
  • Collaborate actively with interdisciplinary healthcare teams, developing tailored treatment plans while coordinating necessary referrals to specialty and social services, including entitlement programs.
  • Facilitate therapeutic services such as supportive and grief counseling, crisis intervention, and psychoeducation, empowering patients and families to navigate the challenges presented by illness and treatment.
  • Independently manage a caseload with varying levels of clinical acuity, engaging in case finding and interdisciplinary round discussions to optimize patient support.
  • Supervise and guide MSWs and student interns by providing individual and group clinical supervision, promoting professional development within the field of social work.
  • Participate in quality assurance initiatives and community improvement projects, contributing to the establishment and refinement of written policies related to social work services.
  • Utilize diagnostic skills as necessary to incorporate mental health diagnoses into treatment planning, ensuring appropriate care strategies are developed.
