• Conduct thorough psychosocial assessments through patient interviews, observations, and medical record evaluations to identify emotional, social, and environmental factors impacting recovery.
• Provide compassionate counseling to patients and their families, addressing adjustment issues and support their emotional health as they navigate rehabilitation.
• Work collaboratively with an interdisciplinary rehabilitation team to develop and implement individualized treatment plans that address patients’ psychosocial needs.
• Guide patients in accessing essential community resources, including support groups, vocational rehabilitation programs, and housing assistance, to facilitate their autonomy and community reintegration.
• Educate patients and their families about available community resources and post-rehabilitation services to empower them in maintaining their health and well-being after discharge.
• Participate actively in discharge planning processes to ensure a smooth transition to home or other care settings, addressing potential challenges and planning for ongoing support.

The clients we serve range from individuals recovering from strokes to those rehabilitating from spinal cord injuries, orthopedic surgeries, and neurological disorders. You will play a crucial role in improving their emotional and social well-being during a particularly vulnerable time in their lives. Your interventions can significantly impact their ability to reintegrate into the community and enjoy a fulfilling quality of life post-rehabilitation.

• A Master’s degree in Social Work from an accredited program is essential for this role.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Demonstrated knowledge of adjustment issues, grief responses, and effective coping strategies vital to supporting patients through their rehabilitation journey.
• Exemplary communication and interpersonal skills to engage collaboratively with patients, families, and the healthcare team.

• Prior experience working in rehabilitation or healthcare settings is favored, equipping candidates with the necessary insight into the unique challenges faced by patients undergoing various rehabilitation processes.
