Job Summary: We are seeking a highly skilled and compassionate LPN to join our dynamic healthcare team by the next quarter. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services.
Requirements:
  • Licensed Practical Nurse (LPN) with active license
  • 3-5 years of current experience helping patients manage chronic conditions (Required)
  • 2-5 years of care management experience with SDoH and behavioral health
  • Current experience and knowledge of patient education methods (Required)
  • Must have effective telephone interaction skills to engage patients successfully
  • EMR and Patient Management Platform use and experience (Required)
Responsibilities:
  • Work with an interdisciplinary care team to engage patients telephonically
  • Perform assessments and identify all problems that affect health and wellness
  • Assist patients telephonically to create and implement person-centered care plans
  • Ensure compliance with all clinical documentation requirements
  • Complete medication reconciliation and coordinate with specialists and other providers
  • Ensure closure of the referral loop
  • Eliminate barriers to care through the case management process and person-centered planning
  • Assist patient with scheduling appointments with the primary care physician and specialists
  • Connect the patient to necessary resources achieve health
  • Conduct Interdisciplinary Care Team Conferences.
