Work Environment

Key Responsibilities

• Conduct thorough mental health assessments for patients and their families, considering past experiences, current challenges, and specific risks related to end-of-life issues.
• Develop and execute personalized treatment plans aimed at addressing mental health concerns, including grief and anticipatory loss, while promoting emotional resilience during a vulnerable time.
• Utilize evidence-based therapeutic techniques, such as cognitive-behavioral therapy (CBT) and supportive counseling, to facilitate coping strategies for patients and families facing terminal illnesses.
• Facilitate individual and family therapy sessions to help clients express their feelings, cope with significant changes, and manage their emotional responses to the dying process.
• Respond to crisis situations by providing immediate support and interventions, including de-escalation strategies and referrals for more intensive support if required.
• Deliver psychoeducation to patients and families about grief processes, mental health resources, and coping mechanisms tailored to their circumstances.
• Maintain comprehensive documentation of assessments, treatment plans, and progress notes in compliance with legal and ethical standards, utilizing electronic health records (EHR) systems.
• Participate in regular team meetings to discuss patient progress, explore collaborative care strategies, and contribute to the overall improvement of service delivery.

Patient Population / Client Focus

The LCSW will work with a diverse range of patients facing serious, life-limiting illnesses alongside their families, providing support that encompasses psychological, emotional, and social facets of care. Understanding the unique challenges of hospice care is crucial, as clients may be dealing with heightened anxiety, depression, and grief associated with their diagnoses.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Nevada.
• A minimum of two years of post-licensure experience in a mental health or hospice environment, preferably within a remote care model.
• Extensive knowledge of mental health disorders, evidence-based treatment approaches, and trauma-informed care principles.
• Demonstrable experience in conducting assessments and formulating treatment plans focused on emotional well-being.
• Proficient in delivering both individual and family therapy, employing various therapeutic modalities.
• Strong crisis intervention and conflict resolution skills, with a capacity to remain composed under pressure.
• Excellent communication and interpersonal skills, enabling rapport building with clients from varied backgrounds.

Preferred Experience

• Experience working with end-of-life care patients and their families.
• Familiarity with telehealth platforms and remote collaboration tools.

Compensation and Benefits
