• Conduct in-depth assessments of patients’ mental health, exploring emotional, behavioral, and social functioning to identify treatment needs accurately.
• Develop and implement individualized treatment plans that are evidence-based, culturally sensitive, and tailored to meet the specific needs of each patient.
• Provide psychotherapy and therapeutic interventions for diverse mental health conditions, including anxiety, depression, and PTSD, utilizing techniques such as cognitive behavioral therapy (CBT) and trauma-informed care.
• Collaborate closely with multidisciplinary healthcare teams, including psychiatrists and primary care providers, to ensure comprehensive and coherent patient-centered care.
• Continuously monitor and evaluate patient progress through regular follow-up sessions, adjusting treatment plans based on patient outcomes and feedback.
• Educate patients and their families about mental health conditions, available treatment options, and community resources to empower them in their care journey.
• Ensure adherence to ethical and professional standards of practice, including confidentiality and informed consent procedures throughout the therapeutic process.

In this key role, you will primarily work with individuals facing terminal illnesses and their families, providing emotional support, counseling, and practical guidance. Your expertise will be essential in addressing the psychological burden associated with end-of-life care, as well as facilitating the grieving process for loved ones. • A Master’s degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) in New York is necessary.

• Exceptional communication and interpersonal skills, with proven ability to build rapport with patients and collaborate effectively with healthcare professionals.

• Experience with electronic health records (EHRs) and familiarity with the use of teletherapy platforms to manage patient information and coordinate care efficiently.
• A comprehensive understanding of mental health conditions, treatment modalities, and the unique challenges faced by patients in hospice care.

While not mandatory, preferred candidates may possess additional experience working in hospice or palliative care settings, as well as training in trauma-informed care approaches. Familiarity with grief counseling techniques will be advantageous in supporting patients and families effectively.
