Key Responsibilities- Conduct in-depth mental health assessments to evaluate the psychological effects of patients’ medical conditions.
- Design and execute individualized treatment plans that address both psychiatric and medical needs, collaborating with an interdisciplinary team.
- Facilitate engaging individual and group therapy sessions, employing evidence-based therapeutic modalities tailored to each patient’s unique situation.
- Work closely with medical providers to ensure that treatment is effective and holistic, enhancing patient outcomes across various healthcare domains.
- Provide immediate crisis intervention and support, equipping patients with coping strategies during mental health emergencies.
- Educate patients on the intrinsic relationship between their mental and physical health, fostering autonomy and effective self-management of their conditions.

Qualifications- Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
- Current licensure as an LCSW, LCPC, or LMFT within the state of Illinois.
- Demonstrated proficiency in managing mental health needs, particularly in individuals with complex medical histories.
- Experience in delivering integrated care and effectively collaborating with medical personnel.
- Exceptional interpersonal and communication skills to build rapport with clients, families, and team members.
- Commitment to maintaining professionalism, adhering to ethical guidelines, and ensuring the confidentiality of patient information.
- Familiarity with electronic health record (EHR) systems and essential computer applications is essential.
- A minimum of two years of practical experience in a clinical mental health context.
- Background in working with patients experiencing co-occurring psychiatric and medical conditions.
