Work Environment

As part of your role, you will interact with a hospice care facility, conducting client assessments and delivering interventions tailored to their unique needs. Key Responsibilities

• Conduct thorough assessments to identify clients' social, emotional, and environmental challenges, ensuring a comprehensive understanding of their situations.
• Develop customized service plans that set specific goals and outline systematic interventions, monitoring progress along the way.
• Provide individual and family counseling sessions to assist clients in coping with grief, loss, and emotional conflicts that arise during the hospice stage.
• Connect clients with necessary community resources such as financial assistance, support groups, and healthcare services, ensuring they have access to a robust network of support.
• Offer crisis intervention services, including immediate risk assessments and safety planning, particularly for families facing sudden changes.
• Engage with local community agencies to coordinate additional services and advocate on behalf of clients’ needs.
• Participate in regular team meetings, case conferences, and staff training to foster continuous professional development and enhance team collaboration.

Patient Population / Client Focus

In this role, you will work primarily with individuals and families in hospice care settings. These clients often face complex emotional needs as they navigate terminal illnesses and associated grief. A large part of your work will involve developing therapeutic alliances with clients, fostering an environment of trust and safety as they process their experiences and prepare for the end-of-life journey.

Qualifications

• A Master’s Degree in Social Work from an accredited institution.
• Current licensure as a Licensed Master Social Worker (LMSW) in the state of Georgia.
• Strong knowledge of social and economic justice issues along with trauma-informed care practices.
• Excellent communication skills, demonstrating cultural competence and the ability to engage effectively with diverse populations.
• Problem-solving capabilities, with a proactive approach to addressing client challenges.

Preferred Experience

• At least one year of experience within a social service agency or similar setting is preferred.
• Experience working in hospice or palliative care is highly regarded.
• Familiarity with local community resources and social services available in Powder Springs and surrounding areas.

Compensation and Benefits
