In the Health and Social Services Manager role is a remote position in which you will lead a team of care managers, while oversee day-to-day clinical operations and participate in business development activities. You will manage the Home and Community Based Services program for the Northern Virginia region, including team and performance management, process improvement, ensure standardized execution of clinical processes, and implementation of new program initiatives. Complying with contractual requirements and ensuring member needs are met.
*While Role is Remote, for consideration candidate MUST reside within the state of VA. Candidates outside of VA will not be considered for this position. *
Primary Responsibilities:
  • Selects, manages, develops, mentors and supports staff in designated department or region
  • Develops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for results
  • To meet the unique needs of our members, have an intimate understanding of the contractual requirements
  • Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements.
  • Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence
  • Acts as regional interface with other departments to coordinate workflow processes and implementation plans.
  • Accountable owner to deliver timely and quality state escalations for complex member situations
  • Ability to work and collaborate effectively across multidisciplinary clinical teams
  • Ensure compliance of your team for processes, regulations, documentation, and training
  • Ability to lead change within your team to increase engagement and foster improvements

Required Qualifications:
  • Bachelor's degree
  • Registered Nurse or Licensed Clinical Social Worker (LCSW) with active and unrestricted license in the state of VA or qualifying reciprocal license
  • 5+ years of experience working within the community health setting in a health care role for HCBS LTSS Members
  • Experience coaching or mentoring staff
  • Leadership experience over programs or projects with clinical team members
  • Intermediate level of experience with Microsoft Word, with the ability to navigate a Windows environment
  • Driver's License and access to a reliable transportation
  • Must live within Virginia and within commutable driving distance to Northern Virginia area.

Preferred Qualifications:
  • Master's degree
  • Certified Case Manager (CCM)
  • 3+ years of leadership experience
  • A background in managed care and dual eligible members in Virginia
  • Case Management experience
  • Experience/exposure with discharge planning
  • Experience in utilization review, concurrent review and/or risk management
