Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify each clientโ€™s mental health concerns, strengths, and available resources.
  • Facilitate individual, family, and group therapy sessions, utilizing evidence-based treatments such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
  • Develop and implement individualized treatment plans, tailoring goals and interventions to meet each client's specific needs and preferences.
  • Collaborate with psychiatrists, psychologists, and other healthcare professionals to ensure seamless, coordinated care for all clients.
  • Provide immediate crisis intervention and risk assessment, including safety planning tailored to client circumstances.
  • Maintain accurate documentation of client progress and treatment interventions, ensuring compliance with ethical and legal standards.
  • Lead group therapy sessions and psychoeducational classes, fostering a supportive community where clients can share experiences and learn from one another.
Patient Population / Client Focus

The clientele served by our hospice center encompasses a diverse range of age groups and backgrounds, many of whom are grappling with advanced illness and the accompanying emotional challenges. Your role will involve working closely with clients who are in vulnerable states, offering compassionate support to improve their mental well-being during crucial moments of their lives. Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of two years of relevant experience in mental health or outpatient settings.
  • Proven expertise in evidence-based therapeutic modalities and treatments.
  • Strong relational skills, with effective communication and thorough assessment capabilities.
