• Conduct comprehensive clinical assessments to identify patient needs, utilizing evidence-based practices to formulate personalized treatment plans.

• Provide solution-focused therapeutic interventions tailored to address the challenges faced by patients and their families in the hospice setting.

• Maintain meticulous and timely clinical records in adherence to HIPAA regulations, ensuring all documentation supports compliance requirements.

• Collaborate closely with a multidisciplinary team, including doctors, nurses, and other social workers, to ensure integrated care for patients.

• Participate in case consultations and peer review meetings to enhance treatment methodologies and improve patient outcomes.

The LCSW will work primarily with patients facing terminal illnesses and their families, providing emotional support, counseling, and interventions aimed at improving the quality of life during critical stages. Our patient population is diverse, including individuals from various socio-economic, cultural, and ethnic backgrounds. You will be instrumental in addressing not only the psychological aspects of their conditions but also assisting families as they navigate complex emotional dynamics during this challenging time.

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW license in the state of Tennessee.

• A minimum of 2 years of clinical experience in social work, specifically within settings that provide psychotherapy.

• Proficiency in solution-based psychotherapy methods, with a strong understanding of mental health challenges in hospice care.

• Exceptional communication and interpersonal skills, with the ability to engage compassionately with patients and families.

• 3 years or more of specialized clinical experience, particularly in hospice or palliative care settings.

• Demonstrated expertise in behavioral health interventions related to grief and loss.

• Experience working with underserved populations and navigating ethical dilemmas in end-of-life care.
