This role is 100% remote, allowing the Licensed Clinical Social Worker the flexibility to perform duties from home while maintaining urgent connectivity with our hospice team. The LCSW will engage in regular virtual meetings, case consultations, and team collaborations to support our patients and their families effectively. • Conduct comprehensive psychosocial assessments for patients entering the hospice program, identifying emotional, social, and financial needs that may impact their care.
• Collaborate closely with the medical teams to develop integrated plans of care that emphasize a holistic approach.
• Facilitate individual and group counseling sessions focusing on grief, coping, and adjustment issues related to terminal illness.
• Coordinate with community service organizations to secure post-discharge services, ensuring a seamless transition for patients and families.
• Provide education and emotional support to patients and their loved ones regarding treatment options, anticipatory grief, and community resources available to them.
• Participate in decision-making committees within the hospice center, offering valuable insights on social work matters that enhance service delivery.
• Maintain accurate, timely documentation in compliance with relevant policies and legal standards, ensuring accountability and clarity in patient care.
• Implement and manage effective discharge planning that aligns patient needs with appropriate community resources.

The Licensed Clinical Social Worker will primarily serve patients in hospice care who are facing terminal illnesses. This often includes individuals from diverse backgrounds with varying needs, such as support for end-of-life decision-making, grief counseling for families, and social resource navigation. The LCSW’s compassionate intervention is critical in alleviating suffering and promoting dignity in patients' final days.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Illinois.

• A minimum of two years of experience working within healthcare or hospital settings, preferably in hospice or palliative care.
• Knowledge and understanding of medical terminology, mental health disorders, and crisis intervention strategies.
• Demonstrated ability to communicate effectively, with strong interpersonal skills to foster collaboration with multidisciplinary teams.
