As a Licensed Clinical Social Worker at Redesign Counseling Hub, you will have the following key responsibilities:

• Collaborate closely with interdisciplinary healthcare teams to develop comprehensive treatment plans that address both psychosocial and clinical needs, including discharge planning.
• Engage with community leaders and resources to advocate for our patients while representing our organization at relevant meetings and committees focused on addressing social needs.
• Provide individual and group counseling, including conducting crisis interventions and assessments to effectively address emotional and behavioral challenges faced by patients.
• Maintain open communication channels with physicians and other healthcare professionals to report on patient progress, including changes in needs and concerns.
• Document all clinical interactions and progress in a timely manner, adhering to our department's policies and standards for record-keeping.

At Redesign Counseling Hub, you will serve a diverse patient population, including children, adolescents, adults, and families facing a variety of mental health challenges. Our clients come from various backgrounds and may be experiencing issues such as anxiety, depression, trauma, and relationship difficulties. Your role will not only involve diagnosing and treating these conditions but also empowering clients to engage in their therapeutic process actively.

To qualify for this role, candidates must meet the following educational and professional requirements:

• A Master’s degree in Social Work (MSW) from an accredited institution is essential, along with necessary licensure as an LCSW.
• A minimum of three years of relevant experience in an outpatient setting is required.
• Demonstrated ability to cultivate and maintain professional relationships with therapists, medical professionals, and patients alike.

In addition to the above qualifications, we prefer candidates who exhibit:

• Strong knowledge of various clinical theories, assessment tools, and evidence-based treatment modalities.
