You will engage virtually with clients and fellow team members, utilizing telehealth technologies and digital platforms to maintain effective communication and coordination of care. • Provide individualized clinical assessments and treatments for clients struggling with major mental illness or severe emotional disorders, both in-office and within the community.
• Create, implement, and monitor personalized care plans, ensuring client access to necessary services while facilitating communication among clients, their families, and other providers.
• Conduct evidence-based screenings and develop thorough assessments that address clients' unique needs, providing targeted interventions.
• Facilitate individual, group, and family therapy sessions, focusing on clients’ presenting problems and service requests.
• Engage in crisis intervention as required, providing immediate support and strategies to help clients navigate acute challenges.
• Maintain accurate and timely documentation for client meetings, treatment progress, and outcome assessments, ensuring compliance with professional ethical standards.
• Monitor client admissions, continued healthcare needs, and discharge planning to optimize treatment outcomes while ensuring safety and quality care.
• Participate actively in team meetings, contributing to collaborative objectives and quality improvement initiatives that enhance service delivery.

The LCSW will work closely with a diverse clientele, primarily consisting of individuals from under-resourced communities facing substance use issues, mental health crises, and co-occurring disorders. You will prioritize relational trust and a trauma-informed approach, crucial for fostering recovery and promoting resilience among clients who may have experienced significant life challenges.

• Master’s degree in Social Work from an accredited institution, with current State licensure as a Licensed Clinical Social Worker (LCSW).
• Robust understanding of assessment, diagnosis, and treatment for clients with severe mental health issues.
• Exceptional verbal and written communication abilities, necessary for delivering effective client care and interacting with multidisciplinary teams.
• Proficient organizational and planning skills, essential for managing a caseload efficiently and ensuring timely follow-ups.

• Previous experience working in substance abuse treatment and detox programs, demonstrating applicable knowledge of the field.
• Experience utilizing electronic health records (EHR) systems for documentation and client management.
• Ability to navigate various multi-disciplinary teams and collaborate effectively with different professionals in the healthcare setting.

The position offers a competitive annual salary range of USD 70,000 to 85,000, commensurate with experience.
