• Conduct thorough psychosocial assessments that gather critical information on clients' mental health concerns, strengths, and available resources.
• Deliver individual, family, and group therapy sessions utilizing evidence-based interventions tailored to the needs of hospice clients.
• Implement therapeutic modalities, including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed therapy, to promote resilience and coping strategies.
• Collaborate with multidisciplinary team members—such as psychiatrists, psychologists, and care coordinators—to create and maintain comprehensive, client-centered treatment plans.
• Facilitate crisis intervention, safety planning, and risk assessments to ensure client safety during vulnerable moments.
• Document clinical interactions and treatment progress accurately and promptly in accordance with ethical guidelines and legal compliance.
• Organize and lead group therapy sessions, psychoeducation classes, and support groups aimed at empowering clients and promoting communal healing.

This role primarily serves clients within a hospice context, where individuals and their families often face terminal illness and the complexities of end-of-life decisions. The social worker will provide support through grief counseling, emotional support, and education, ensuring that every client receives compassionate care that respects their dignity and personal preferences.

• A Master’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) in Oregon.
• A minimum of two years of experience providing mental health services, preferably in outpatient or hospice settings.
• Proficiency in a variety of evidence-based therapeutic techniques and interventions.
• Outstanding interpersonal and communication skills, enabling effective client rapport and collaboration with treatment teams.

• Expertise in working with diverse populations, including individuals from various cultural and socioeconomic backgrounds.
• Experience with grief and bereavement counseling is an asset.
• Familiarity with telehealth platforms and remote counseling practices.
