This full-time remote position focuses on providing telehealth support to our senior clients, ensuring they receive tailored mental health care and psychosocial assistance. This role is fully remote, leveraging telehealth technologies to provide critical support to seniors in a comfortable and accessible setting. Our team employs advanced telecommunication tools to ensure effective interaction with clients and their families. The work hours are generally Monday to Friday from 8:00 AM to 4:30 PM, with some flexibility required for evening or weekend hours based on client needs. On-call availability may also be necessary to manage urgent situations.

• Conduct comprehensive assessments to identify the psychosocial needs of seniors, taking into account emotional, mental, and social factors.
• Develop and implement personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.
• Provide emotional support and counseling to seniors and their families, assisting them with life transitions such as aging, loss, or chronic illnesses.
• Advocate for clients’ access to community resources and services, ensuring they receive the necessary support systems in place.
• Work with interdisciplinary teams, including healthcare professionals and caregivers, to provide coordinated care that addresses the holistic needs of clients.
• Regularly monitor and evaluate clients' living conditions and well-being through scheduled virtual visits and address any concerns that arise.
• Educate clients and families about available community resources, support groups, and engagement activities to foster social interaction and improve quality of life.
• Maintain detailed and accurate documentation of client interactions, assessments, and progress reports compliant with ethical standards and regulations.

This position primarily serves seniors who face unique challenges related to aging, including mental health issues, social isolation, and difficulties in accessing healthcare. By addressing the individual needs of these clients through empathetic communication and targeted interventions, the Senior Social Worker plays an important role in enhancing their psychosocial well-being.

• A Bachelor’s degree in Social Work from an accredited institution is required.
• A valid license as a Social Worker (LCSW) in the state of Georgia.
• Minimum of two years of professional experience in social work, ideally in a community center focused on geriatric care.
• Deep knowledge of social work principles, community resources, and aging-related issues.
• Demonstrated skills in counseling, problem-solving, and conflict resolution.
• Excellent communication skills, both verbal and written, with a strong ability to establish rapport with seniors and their families.
• Proficiency in using electronic health record (EHR) systems and relevant computer applications for documentation and client management.

• Experience working in telehealth environments is a plus.
• Background in geriatric social work or mental health counseling related to aging populations.
• Familiarity with local community resources and services for seniors in Augusta, Georgia.

The salary range for this position is between USD 60,000 and 100,000 annually, commensurate with experience and qualifications.
