• Conduct thorough psychosocial assessments during initial consultations to understand the patient's emotional health landscape, history, and current challenges.
• Provide tailored individual and family therapy sessions, utilizing evidence-based approaches like cognitive-behavioral therapy (CBT) and mindfulness strategies to help patients cope with grief and anxiety related to end-of-life issues.
• Develop comprehensive treatment plans that incorporate personal and family goals, actively involving patients and their families in the therapeutic process.
• Collaborate closely with an interdisciplinary team, including physicians and nurses, to devise and implement coordinated care plans tailored to the unique needs of hospice patients.
• Facilitate group therapy sessions and psychoeducational workshops aimed at equipping families with effective coping strategies and support resources.
• Provide crisis intervention services, including risk assessments and safety planning, ensuring that immediate emotional crises are addressed promptly and effectively.
• Maintain precise documentation that meets ethical and legal regulations, ensuring continuity of care and efficient treatment outcome tracking.

Your primary clientele will consist of hospice patients and their families, facing various mental health issues related to terminal illness, including depression, anxiety, and grief. You will play a crucial role in providing emotional support and therapeutic interventions that can significantly enhance the quality of life and mental well-being for both patients and their loved ones during this sensitive phase.

• A Master's degree in Social Work from an accredited institution.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
• Minimum of two years of practical experience in mental health, particularly within an outpatient or hospice setting.
• Demonstrated understanding of evidence-based therapeutic modalities and treatment frameworks.
