We are currently seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our healthcare team in Greenville, TX. The ideal candidate will have a strong background in patient care, with a preference for those experienced in Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) care.

Position Overview

As an LVN on our team, you will provide high-quality nursing care to patients, assist with care coordination, and support remote monitoring initiatives to improve patient outcomes. This role requires excellent clinical skills, attention to detail, and the ability to work collaboratively within a multidisciplinary team.

Key Responsibilities
  • Provide direct nursing care to patients in accordance with established protocols and standards.
  • Assist with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, including patient education and monitoring.
  • Document patient information accurately and timely in electronic health records.
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
  • Monitor patient conditions and report any changes or concerns promptly.
  • Support patient engagement and adherence to treatment plans through effective communication.
Qualifications
  • Current and valid LVN license in the state of Texas.
  • Experience with Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) preferred.
  • Strong clinical assessment and patient care skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of a team.
  • Familiarity with electronic health records (EHR) systems.
