Wise Mind Psychotherapy Services is seeking a dedicated and passionate Licensed Clinical Social Worker (LCSW) to join our team. This full-time position offers the flexibility of a remote work setting while providing vital services within a hospice center located in Chattanooga, Tennessee. As an LCSW, you will play a crucial role in delivering high-quality mental health services to clients dealing with significant mental health challenges, particularly in the context of terminal illness care.

This remote position is designed to offer a supportive and collaborative work environment while giving our Licensed Clinical Social Workers the flexibility to engage with clients from the comfort of their own homes. The role involves direct interaction with patients and their families, as well as collaboration with an interdisciplinary team of healthcare providers, ensuring comprehensive care coordination. • Engage directly with clients experiencing major mental illnesses and severe emotional disorders, providing clinical intervention in both individual and group settings.
• Create and implement integrated care plans that ensure access to necessary resources and services for clients, while also communicating effectively with clients, their families, and other healthcare providers.
• Conduct comprehensive screenings and assessments to determine treatment needs and create individualized care strategies based on evidence-based practices.
• Monitor and document client progress, including managing intake processes, ongoing treatment evaluations, and discharge planning.
• Participate in interdisciplinary team meetings to discuss client care, collaborate on treatment strategies, and support shared team goals.
• Assist in crisis intervention efforts when necessary, employing de-escalation techniques and appropriate resources to support clients in distress.
• Complete timely and accurate documentation of client interactions and treatment outcomes, following ethical guidelines and organizational protocols.
• Provide on-call support during and after program hours as assigned to ensure all clients have access to necessary services.

The patients you will serve primarily include individuals facing terminal illnesses and the accompanying mental health challenges associated with grief, loss, and anxiety. Our focus is on providing holistic care that encompasses the psychological, social, and emotional aspects of hospice care, helping our clients navigate their circumstances with dignity and support.

• A Master's Degree in Social Work from an accredited institution.
• Active LCSW license in the state of Tennessee.
• A minimum of 2 years of clinical experience in mental health and social work, preferably in hospice or palliative care settings.
• Strong understanding of the ethical and legal considerations related to social work practice.

• Experience working with diverse populations and understanding of cultural sensitivity and competency.
• Familiarity with integrated care models and interdisciplinary collaboration within the mental health and healthcare fields.
• Proficient in utilizing electronic health record (EHR) systems for documentation and client management.

The salary for this full-time Licensed Clinical Social Worker position ranges from USD 70,000 to 85,000 per year, commensurate with experience and qualifications.
