Your day-to-day responsibilities will include:

• Conducting thorough psychosocial assessments to evaluate patient needs and challenges, focusing on emotional, social, and financial aspects.
• Providing both individual and group counseling sessions aimed at helping patients cope with changes, grief, and recovery processes.
• Coordinating with community organizations to facilitate post-discharge services, ensuring patients receive necessary support after leaving the detox center.
• Offering consistent emotional support and education to patients and their families about treatment options, resources available, and coping strategies.
• Participating in weekly multidisciplinary team meetings, bringing insights on social work-related matters that enhance patient outcomes.
• Implementing crisis intervention when necessary, ensuring patients and families feel secure and supported during challenging times.
• Developing and executing comprehensive discharge plans that connect patients with relevant community resources and support systems.
• Maintaining accurate and timely documentation in compliance with legal standards and organizational policies, contributing to effective care continuity.

Our detox center serves a diverse patient population struggling with substance abuse and co-occurring mental health disorders. • A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Connecticut.

• Excellent communication, interpersonal, and collaboration skills to work effectively in a remote interdisciplinary environment.

While not mandatory, the following experiences are preferred:

• At least two years of experience in a healthcare or hospital setting, preferably within substance abuse treatment or a related area.
• Familiarity with crisis intervention techniques and evidence-based practices in social work.
