Key Responsibilities
  • Conduct comprehensive evaluations and assessments to determine the developmental needs of young children.
  • Engage actively in the Individualized Family Service Plan (IFSP) development and implementation process, tailoring strategies to meet each child's unique needs.
  • Monitor and evaluate outcomes, collaborating closely with a multidisciplinary team to ensure that children receive effective and evidence-based interventions.
  • Provide essential support to families, equipping them with practical strategies and resources to enhance their child's development.
  • Maintain compliance with all federal and state regulations, adhering to best practices in social work and early intervention.
  • Foster rapport and trusting relationships with children, caregivers, and team members to create a nurturing and supportive learning environment.
  • Utilize Natural Learning Environment Practices to implement the IFSP effectively, adjusting service frequency according to the child's developmental status and family priorities.
  • Deliver information and training to families, empowering them with the skills necessary to support their child's growth.

  • Uphold high standards of health and safety for all staff and individuals served.
Patient Population / Client Focus

Qualifications
  • Master's Degree or Doctorate in Social Work, with a valid license issued by the Department of Public Health.
  • Current LCSW designation is a requirement for this role.

  • A minimum of one year of experience working in early intervention settings, directly supporting children and families.
