• Conduct comprehensive intake assessments to evaluate patients’ mental health needs, ensuring the development of personalized, client-centered treatment plans.
• Facilitate individual therapy sessions through virtual platforms, employing evidence-based therapeutic modalities tailored to each client’s unique situation.
• Coordinate care with interdisciplinary teams, including physicians and nurses, to ensure a holistic approach to patient well-being and treatment.
• Utilize HIPAA-compliant electronic health records to document client interactions and treatment progress accurately, ensuring adherence to regulatory and ethical standards.
• Provide ongoing support to patients and families, including crisis intervention and grief counseling, to navigate complex emotional landscapes during the hospice journey.

You will primarily serve a diverse patient population with varied cultural backgrounds and mental health needs, each requiring a personalized approach to care. Your role is critical in addressing not only the psychological needs of patients but also the emotional needs of their families during difficult times. This diverse client focus demands a strong cultural competency to effectively communicate and provide tailored support in sensitive situations.

• A Master’s degree in Social Work or a related field from an accredited institution.
• An active and unrestricted Licensed Clinical Social Worker (LCSW) license in the state of California.
• A minimum of two years of direct experience in a clinical social work setting, particularly with hospice or palliative care populations.
• Demonstrated expertise in treatment planning, therapeutic modalities, and documentation of clinical interactions in a healthcare setting.
• Strong interpersonal skills combined with the ability to engage with clients from a variety of cultural backgrounds, fostering a trusting therapeutic relationship.

• Experience working in a hospice environment or with terminally ill patients.
• Familiarity with electronic medical record systems and telehealth practices.
• Advanced training in grief counseling, trauma-informed care, or relevant certifications.

The salary for this role ranges from $110,000 to $130,000 annually, commensurate with education and experience, accompanied by competitive health, dental, and vision insurance.
