Key Responsibilities

As an LCSW in our remote hospice care setting, you will:

  • Conduct comprehensive psychosocial assessments for individuals dealing with terminal illnesses, ensuring that treatment plans are tailored to meet their emotional and mental health needs.

  • Implement and administer a variety of evidence-based therapeutic interventions, including but not limited to cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored specifically to hospice patients.

  • Maintain thorough and accurate clinical documentation in accordance with HIPAA standards, ensuring timely updates to patient records and treatment plans.

  • Engage in multidisciplinary team meetings with physicians, nurses, and other healthcare providers to optimize patient care and enhance treatment outcomes through coordinated efforts.

  • Provide guidance and education to patients and their families regarding coping strategies, available community resources, and support systems.

  • Participate in regular case consultations and peer review sessions, sharing insights and enhancing your skills while contributing to a culture of continuous improvement.

  • Stay informed about the latest advancements in hospice care and mental health treatments, integrating this knowledge into your clinical practice for improved patient outcomes.

Patient Population / Client Focus

You will primarily work with patients facing terminal illnesses and their families, guiding them through complex emotional landscapes. Your expertise in navigating grief, loss, and despair is critical in helping our clients transition with dignity. Your role directly supports their emotional well-being and helps families cope during one of the most challenging times in their lives.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.

  • A valid LCSW license to practice in the state of Arizona.

  • At least 2 years of clinical experience, particularly in hospice, palliative care, or similar settings, providing psychotherapy services.

  • Demonstrated proficiency in solution-focused psychotherapy techniques.

  • Exceptional communication and interpersonal skills for effective patient interaction and collaboration with a multidisciplinary team.

Preferred Experience
