Heritage Wellness Clinic is seeking a dedicated Licensed Clinical Social Worker (LCSW) for a full-time, remote position within our Hospice Center in Fort Worth, Texas. Daily tasks will include assessments, therapy sessions, and collaboration through various electronic health record systems, ensuring that quality care is delivered seamlessly.

• Conduct thorough assessments and when necessary, diagnose patients using clinically accepted tools, such as the Diagnostic and Statistical Manual of Mental Disorders, to inform and create individualized treatment plans that honor cultural diversity and address various special needs.
• Continually monitor and evaluate patient progress, offering targeted interventions and strategies that align with overarching mental health diagnoses.
• Prepare, maintain, and monitor all clinical documentation, ensuring compliance with regulatory standards while utilizing current technology effectively.
• Work collaboratively with patients and their support systems to craft comprehensive treatment plans that encompass patient needs and desired outcomes.
• Facilitate patient safety and symptom management, actively engaging in teamwork that promotes open communication and shared responsibilities.
• Coordinate referral pathways, serving as a vital link between patients and other necessary treatment providers, thus maintaining strong professional networks.
• Provide guidance and mentorship to students, interns, and volunteers, fostering an environment of learning and professional development.
• Engage in peer reviews and collaborative clinical discussions under the direction of a clinical supervisor to aid in the growth and development of team members.

This position focuses on working with individuals and families grappling with the emotional and psychological impacts of cancer diagnoses. Patients may exhibit a range of mental health needs, demanding an empathetic and well-informed approach from their social worker. The LCSW will tailor therapeutic interventions to address not only the clinical aspects of mental health but also the unique experiences of each patient, encouraging resilience and emotional stability throughout a challenging journey.

• A Master's degree in Social Work from an accredited program.
• A valid and current state license as a Licensed Clinical Social Worker (LCSW).

• Experience in hospice or palliative care settings, particularly with adult and geriatric populations facing terminal illness.
• Familiarity with evidence-based therapeutic practices tailored to cancer patients.
• Previous experience collaborating with interdisciplinary teams and managing complex patient cases.

The salary for this full-time role ranges from USD 90,000 to 105,000 annually, commensurate with experience and qualifications.
