Key Responsibilities
  • Conduct comprehensive clinical assessments to evaluate the mental health needs of patients, formulating tailored treatment plans that address individual concerns.
  • Implement evidence-based therapeutic interventions, including individual and family therapy, utilizing approaches that are effective in hospice and palliative care.
  • Document the clinical progress and treatment outcomes in an organized and timely manner within our electronic health record (EHR) system.
  • Collaborate with a multidisciplinary treatment team consisting of physicians, nurses, and other healthcare professionals to ensure holistic care delivery.
  • Provide education and emotional support to patients and families about coping strategies, resources, and end-of-life issues, facilitating open communications to enhance understanding.
  • Stay informed on the latest research and advancements in mental healthcare to continuously enhance service quality at our hospice center.
Patient Population / Client Focus

Your work will involve engaging with a diverse range of patients, including those with terminal illnesses, their families, and caregivers. This role is centered around empowering these individuals to express their feelings, fears, and wishes, ultimately guiding them through one of life’s most vulnerable stages.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • A valid LCSW license in the state of Florida.
  • Strong foundation in solution-based therapy interventions, particularly within the context of grief, loss, and end-of-life care.
  • Exceptional communication and conflict-resolution skills, enabling effective engagement with patients and interdisciplinary colleagues.
Preferred Experience
  • A minimum of two years of relevant clinical experience, ideally in hospice or palliative care settings.
  • Familiarity with telehealth platforms and remote counseling techniques.
  • Experience working with diverse populations and understanding cultural competencies in care.
