Location: Palm Beach Island, FL
Part Time/PRN (Must have weekend availability)
Overview:
We are a Medicare-certified home health agency seeking a dedicated and skilled Per Diem Licensed Practical Nurse (LPN) to deliver quality, compassionate care to patients in their homes. The LPN will work collaboratively with RNs and other members of the interdisciplinary team to implement the established plan of care and support patients’ health goals in the comfort of their homes across our service territory.
This position offers flexible scheduling, autonomy, and the opportunity to build meaningful relationships with patients and families while providing one-on-one nursing care.
Responsibilities:
Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the physician's plan of care.
Perform nursing treatments including wound care, medication administration, catheter care, and monitoring of vital signs.
Observe and report changes in the patient's condition to the RN Case Manager and/or physician promptly.
Educate patients and caregivers on prescribed care, medications, and disease processes as instructed in the plan of care.
Accurately document patient care and observations in the EMR system in a timely manner.
Collaborate with RNs, therapists, aides, and physicians to ensure comprehensive and coordinated patient care.
Travel to patient homes within the designated territory
Per Diem / PRN – Work based on your availability and patient needs.
Competitive per-visit pay based on visit type
Current, active LPN license in the State of Florida.
Minimum of one (1) year of clinical nursing experience; home health experience preferred.
Familiarity with Medicare guidelines and home health documentation standards is a plus.
Proficiency in using an EMR system (experience with Kinnser/WellSky or similar is helpful).
Reliable transportation, valid driver's license, and current auto insurance.
Strong clinical judgment, communication, and time-management skills.
Ability to work independently while following agency protocols and standards.
