Key Responsibilities
  • Conduct comprehensive client assessments to evaluate mental health needs, including the identification of trauma experiences.
  • Develop and implement personalized treatment plans that utilize evidence-based interventions tailored to individual client needs.
  • Maintain accurate, confidential clinical records in compliance with HIPAA regulations and organizational policies.
  • Collaborate with a multidisciplinary team, including medical professionals, nurses, and psychologists, to ensure holistic care for clients.
  • Educate clients and their families about mental health issues, treatment options, and coping strategies, to empower them in their healing journeys.
  • Stay updated on the latest mental health research, best practices, and emerging trends to continually enhance service provision.
Patient Population / Client Focus

As a social worker within our hospice center, you will primarily work with individuals grappling with terminal illnesses, alongside their families. You will address psychological challenges stemming from grief, anxiety about end-of-life transitions, and the emotional impact of loss. The diverse backgrounds of our clients necessitate a culturally competent approach, ensuring that services are relevant and responsive to each unique situation.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • A valid Licensed Clinical Social Worker (LCSW) credential in the state of Tennessee.
  • A strong foundation in solution-focused psychotherapy principles.
  • Excellent communication and interpersonal skills to engage effectively with clients and collaborate with team members.
  • Proven ability to manage complex cases with critical thinking and problem-solving skills.
  • Commitment to providing compassionate care within a collaborative, client-centered framework.
Preferred Experience
  • At least two years of clinical experience related to mental health settings, with an emphasis on trauma and grief counseling.
  • Experience working in hospice care or similar environments is highly desirable.
