Key Responsibilities
  • Conduct comprehensive clinical assessments of patients, identifying their psychological, emotional, and social challenges.
  • Develop and implement individualized treatment plans using evidence-based approaches tailored to each patient’s unique circumstances.
  • Administer a range of therapeutic interventions, including individual and family therapy sessions, focusing on solutions that empower patients.
  • Maintain accurate and timely clinical documentation and treatment summaries, ensuring compliance with organizational standards and state regulations.
  • Collaborate effectively with a multidisciplinary treatment team, including physicians, nurses, and chaplains, to coordinate comprehensive care for hospice patients.
  • Provide education and resources to patients and their families about coping strategies, grief, and mental health support.
  • Engage in ongoing professional development by staying updated with current research and advancements in the field of mental health and social work.
Patient Population / Client Focus

Your work will primarily focus on hospice patients facing terminal illnesses, alongside their families, who require emotional support and guidance. This role involves addressing a diverse range of emotional and psychological needs during some of the most challenging times in their lives. Your capacity to build trust and create meaningful connections will be essential in helping families navigate grief, prepare for end-of-life decisions, and find peace.

Qualifications
  • A Master’s Degree in Social Work (MSW) from an accredited institution.
  • A current, valid Licensed Clinical Social Worker (LCSW) license in the state of Michigan.
  • Knowledge and experience in solution-focused therapy and other effective therapeutic interventions.
  • Exceptional communication, interpersonal, and problem-solving skills.
Preferred Experience
  • A minimum of two years of clinical social work experience, preferably in hospice, palliative care, or mental health settings.
  • Demonstrated ability to work collaboratively within a team-oriented environment and adapt to diverse patient populations.
  • Experience with electronic health record systems to maintain patient documentation and treatment plans efficiently.
Compensation and Benefits

The salary for this position ranges between USD 90,000 and 104,000 annually, commensurate with experience.
