Position Overview

Are you a dedicated and compassionate Licensed Clinical Social Worker (LCSW) looking to make a meaningful impact in the lives of individuals facing life-limiting illnesses? Health Access Medical Center is currently seeking a full-time, fully remote LCSW to join our team as a Hospice Care Specialist. In this role, you will provide essential virtual therapy services to clients, helping them navigate the emotional, social, and psychological challenges associated with hospice care.

Work Environment

This position offers the flexibility of working remotely from anywhere in the United States, allowing you to balance your professional responsibilities with personal commitments. As a fully virtual role, you will engage with clients through secure telehealth platforms, eliminating travel and providing you the opportunity to create your own schedule while managing a minimum caseload of 25 clients per week. Weekend availability is encouraged but flexible, catering to the diverse needs of our patient population.

Key Responsibilities
  • Conduct comprehensive assessments and formulate personalized treatment plans for clients utilizing standard therapeutic techniques.
  • Provide individual counseling sessions through virtual platforms, ensuring a supportive and safe environment for clients.
  • Utilize evidence-based practices, including Cognitive Behavioral Therapy (CBT) and Psychodynamic Therapy, to address the unique emotional needs of clients in hospice care.
  • Maintain accurate documentation of client interactions, treatment progress, and outcomes in our Electronic Health Record (EHR) system for compliance and auditing purposes.
  • Coordinate with interdisciplinary teams, including physicians and nursing staff, to deliver holistic care that addresses both the physical and mental health needs of clients and their families.
  • Participate in regular training and professional development workshops to stay up-to-date with best practices in behavioral health interventions.
Patient Population / Client Focus

Your primary focus will be on providing support to clients aged 18 and older who are experiencing terminal illnesses. This involves addressing complex emotions such as grief, anxiety, and depression that may arise during this challenging phase of life. It is vital to engage with clients from diverse backgrounds, ensuring culturally sensitive approaches that respect individual values and beliefs, particularly in the context of hospice care.

Qualifications
  • A Masterโ€™s degree in Social Work or Mental Health Counseling from an accredited institution.
  • Active LCSW license in the State, with a strong ethical foundation in practice.
  • A minimum of three years of clinical counseling experience, post-licensure, demonstrating proficiency in patient assessment and intervention.
  • Experience providing therapy to individuals in hospice or palliative care settings is preferred but not required.
  • Possession of professional malpractice insurance to safeguard your practice.
Preferred Experience
  • Familiarity with supporting the LGBTQIA+ community and knowledge of men's mental health issues is a plus.
  • Previous experience in providing psychotherapy to minority and diverse populations, showcasing an understanding of various cultural dynamics.
