As part of our remote team, you will participate in regular virtual meetings with healthcare professionals, ensuring collaborative approaches to patient care and ongoing professional development.

• Conduct detailed clinical assessments to evaluate patient mental health needs, particularly focused on coping with terminal diagnoses.
• Develop and implement individualized treatment plans using evidence-based therapeutic interventions tailored to each patient's unique circumstances.
• Facilitate family meetings and support sessions, providing education and guidance to help families cope with emotional and psychological aspects of end-of-life care.
• Maintain accurate, timely clinical documentation in our electronic health record (EHR) system, adhering to HIPAA regulations and organizational policies.
• Collaborate actively with a multidisciplinary team, including physicians, nurses, and other healthcare providers, to align mental health support with overall patient care.
• Engage in continuous professional development by staying informed about the latest research and treatment approaches in mental health and hospice care.

Your patient focus will include a diverse range of individuals, from elderly patients grappling with terminal illnesses to younger patients facing acute health challenges. Each interaction will emphasize empathy, patience, and a deep understanding of the grief process that both patients and families must navigate.

• A Master's degree in Social Work (MSW) from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Arizona.
• A solid understanding of solution-focused and therapeutic interventions relevant to hospice and end-of-life care.
• Strong communication skills, including the ability to convey complex emotional topics with sensitivity and clarity.

• Experience working in hospice settings, palliative care, or an environment focused on end-of-life issues.
• Demonstrated experience in providing counseling to patients and families facing terminal illnesses, including grief and loss support.
• Familiarity with electronic health record systems and clinical documentation standards.

We offer a competitive salary range of USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
