This is a remote position that enables flexibility while promoting a healthy work-life balance. As a member of our dedicated team, you will primarily serve clients from a hospice facility based in Garden Grove, California. You will work collaboratively with a multidisciplinary team of healthcare professionals, ensuring that a holistic approach is taken in addressing the emotional and psychological needs of patients and their families.

• Conduct comprehensive assessments to understand individual client needs and develop tailored care plans that address specific emotional and social challenges.
• Implement evidence-based interventions and methodologies to enhance mental health outcomes among hospice patients, utilizing therapeutic techniques suitable for this unique environment.
• Maintain clear, accurate, and timely clinical documentation, ensuring compliance with industry standards and facilitating effective communication among team members.
• Work closely with doctors, nurses, and other healthcare professionals to coordinate and deliver holistic care, adjusting treatment plans as necessary to best serve patient needs.
• Educate clients and their families on mental health resources, coping strategies, and available support systems, fostering empowerment and resilience during difficult times.
• Stay updated on new treatment methodologies, integrating best practices into your therapeutic approach to ensure the highest quality of care.

In this role, you will focus on providing compassionate support to individuals nearing the end of their life journey and their families. This entails addressing a myriad of emotional responses, including grief, anxiety, and depression, presenting unique therapeutic challenges. You will play a crucial part in supporting clients to navigate complex feelings while equipping their families with tools to cope during this vulnerable time.

• A Master's degree in Social Work from an accredited institution.
• An active LCSW license in the state of California.
• Demonstrated ability to work effectively in high-volume clinical settings, managing priorities and client needs efficiently.
• Adept problem-solver, with the ability to work independently while being a collaborative team player.

• Minimum of two (2) years of clinical experience, particularly in hospice care or palliative settings, is preferred.
• Familiarity with evidence-based therapeutic techniques and approaches in the mental health field.
• Experience working with diverse populations and sensitivity to the cultural implications of caregiving.
