• Conduct thorough psychosocial assessments to understand each client’s mental health challenges, strengths, and support systems.
• Provide individual, family, and group therapy sessions utilizing evidence-based practices such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.
• Develop and implement personalized treatment plans, setting clear, measurable goals and determining appropriate interventions in collaboration with clients.
• Work alongside a multidisciplinary team, including psychiatrists and psychologists, to deliver comprehensive, coordinated care.
• Facilitate crisis intervention strategies, conduct risk assessments, and develop safety plans as necessary to ensure client safety and well-being.
• Document client interactions and treatment progress meticulously, adhering to ethical and legal standards for confidentiality and record-keeping.
• Lead group therapy sessions, psychoeducational workshops, and support groups, fostering community among clients while addressing mental health challenges.

Your primary focus will be on clients receiving hospice care, often facing terminal illnesses and significant emotional distress. You'll engage with patients and their families to navigate complex feelings surrounding grief, loss, and existential concerns, ensuring each client's journey is met with compassion and understanding.

• Master's degree in Social Work from an accredited institution, providing you with foundational knowledge and skills necessary for effective practice.
• Active licensure in the state of New York as a Licensed Clinical Social Worker (LCSW), signifying your eligibility to practice independently.
• A minimum of two years of experience in a mental health or outpatient setting, with preference given to those experienced in hospice care or similar environments.
• Strong familiarity with evidence-based therapeutic modalities and the ability to adapt interventions to suit a diverse range of client needs.
