Key Responsibilities
  • Conduct individual assessments and develop integrated care plans tailored to the needs of each client.
  • Perform brief, evidence-based screenings and coordinate additional more detailed assessments when necessary.
  • Engage with clients in both one-on-one and group therapy settings, fostering an environment that encourages healing and growth.
  • Collaborate with interdisciplinary teams to enhance service delivery, ensuring that treatment plans are holistic and comprehensive.
  • Maintain accurate and timely documentation of client interactions, treatment progress, and outcomes in alignment with organizational standards.
  • Assist with crisis interventions as required, using effective problem-solving and intervention strategies.
  • Participate in quality improvement committees to enhance agency services and outcomes for clients.
  • Provide ongoing support, follow-ups, and referrals to community resources to ensure holistic care for clients.
Patient Population / Client Focus

The Licensed Clinical Social Worker will primarily support school-age children and adolescents facing mental health challenges such as anxiety, depression, trauma, and behavioral disorders. You will be required to create and maintain strong therapeutic relationships with clients, emphasizing empowerment and choice in their treatment process. Qualifications
  • A Bachelor’s Degree in Social Work from an accredited institution.
