Position Overview

Heritage Wellness Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in Colorado Springs, Colorado. In this full-time remote position, the LCSW will provide outpatient therapy to individuals and families profoundly affected by cancer diagnoses. Key Responsibilities
  • Conduct thorough assessments, diagnoses when necessary, and effectively evaluate each patient’s status and progress using clinically appropriate tools, including the Diagnostic and Statistical Manual of Mental Disorders.
  • Create and implement individualized plans of care that respect cultural diversity and address the unique needs of the patient population, particularly those dealing with cancer-related issues.
  • Utilize evidence-based therapeutic modalities to support patient treatment goals, employing strategies that promote mental wellness.
  • Maintain accurate and timely documentation of treatment records, reports, and clinical information in accordance with organizational policies and regulatory requirements.
  • Collaborate with patients and their families to develop comprehensive treatment plans, ensuring alignment with identified goals and desired patient outcomes.
  • Regularly reassess patient progress and modify interventions as needed to ensure ongoing support for their mental health diagnosis.
  • Coordinate referrals and act as a liaison between patients and other treatment providers, establishing strong professional relationships to facilitate comprehensive care.
  • Engage in the education and mentoring of students, interns, and volunteers, fostering an inclusive learning environment for future health professionals.
  • Assist in peer review, clinical collaborations, and other initiatives aimed at strengthening team skills and enhancing care delivery processes.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits

This position offers a competitive salary ranging from $90,000 to $105,000 annually, commensurate with experience and qualifications.
