• Act as a liaison between clients, their families, and outpatient providers, ensuring seamless communication and support throughout treatment.
• Collaborate with insurance companies to navigate pre-certification processes and utilization management, facilitating appropriate care and service continuity.
• Participate actively in discharge planning and aftercare support, setting clients up for success post-treatment through tailored resources and strategies.
• Provide therapeutic meal support, guiding clients to develop a healthy relationship with food and body image in a safe and nurturing manner.
• Deliver empathetic and culturally competent care, adapting your approach based on each client’s unique identities and circumstances.

Your work will primarily focus on serving older adults, many of whom may be experiencing mental health challenges related to aging, such as depression, anxiety, or cognitive decline. You will address their specific needs by employing evidence-based therapeutic modalities and interventions to enhance their quality of life. By building strong therapeutic relationships, you will help clients navigate their emotional landscapes and foster resilience.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of California is mandatory.
• Experience working with older adults is essential to understanding their unique challenges and treatment needs.
• Strong clinical skills in assessment, diagnosis, and intervention with demonstrated ability to implement evidence-based practice.
• Excellent verbal and written communication skills are critical for effective collaboration with clients, families, and multidisciplinary teams.
• Ability to work both independently and collaboratively as part of a cohesive, remote team.

• Experience with electronic health records (EHR) systems for documentation and client management.
• Familiarity with community resources and support services for older adults.
• Training in trauma-informed care practices or related therapeutic modalities is a plus.
