This position will operate primarily in a remote setting, allowing flexibility while ensuring broad access to our comprehensive hospice services. • Serve as a vital liaison between families and outpatient providers to coordinate care and communicate health updates effectively.
• Engage with insurance companies for pre-certification processes and manage utilization reviews to optimize client benefits.
• Participate actively in discharge planning and aftercare discussions, ensuring a smooth transition for clients from hospice services back to community resources.
• Provide therapeutic meal support, guiding clients in developing a healthy relationship with food through empathetic engagement.
• Deliver safe, affirming, and personalized care that respects the unique needs and intersecting identities of every client.

The ideal candidate will work closely with older adults, confronting the complexities of hospice care. This role involves offering emotional and practical support to clients as they navigate end-of-life issues while supporting their families during this challenging time. You will be responsible for conducting assessments that inform care plans and engaging in interventions focused on emotional well-being, coping strategies, and grief processing.

• A Master's degree in Social Work (MSW) from an accredited institution.
• A current and valid LCSW license recognized by the state of Georgia.
• Demonstrated experience working with older adults, particularly in a hospice or palliative care setting.
• Strong clinical and diagnostic skills to effectively assess and address client needs.
• Exceptional communication and interpersonal skills that foster trust and collaboration.
• Ability to work autonomously as well as part of a cohesive team.

• Experience in hospice or end-of-life care settings is highly desirable.
• Familiarity with treatment modalities that are effective for individuals coping with terminal illness and loss.
• Proficiency in electronic health record (EHR) systems to document assessments, care plans, and progress notes.
