Position Overview

We are seeking a dedicated and experienced Clinical Social Worker (LCSW) to join our innovative team at MemorialCare Health System. In this full-time remote role, you will play a pivotal part in enhancing the quality of care provided at our Detox Center in Sunnyvale, California. As a Licensed Clinical Social Worker, you will be responsible for mitigating psychosocial barriers that hinder patients' safe and timely transitions through the continuum of care.

Key Responsibilities
  • Manage a specific caseload of patients, thoroughly evaluating their social, emotional, financial, and cultural circumstances to create comprehensive treatment plans.
  • Conduct assessments and apply various therapeutic modalities aimed at removing psychosocial barriers, thereby facilitating smooth transitions through levels of care.
  • Lead care coordination efforts by facilitating discussions around protective services, advance directives, and complex life care decisions for high-risk patients.
  • Provide crisis intervention, collaborating with healthcare teams to deliver immediate support to patients and families facing urgent psychosocial concerns.
  • Educate patients, families, and caregivers about available resources, ensuring their psychosocial and socioeconomic needs are met during treatment and discharge planning.
  • Supervise and mentor Social Work Interns and staff, conducting weekly supervision sessions to enhance both their professional development and the standards of practice within the department.
  • Actively engage in community outreach, liaising with various agencies to procure resources that prevent hospital readmissions and reinforce effective utilization of funding.
  • Ensure timely completion of psychosocial services to prevent delays in patient care and discharge, while managing complex biopsychosocial cases that require nuanced intervention.
  • Participate in weekly team rounds and patient care conferences, contributing insights that enhance collaborative treatment approaches for diverse patient populations.
  • Be available for on-call duties on a rotational basis, ready to support the healthcare team and address any emergent case concerns.

Qualifications
  • Master's degree in Social Work (MSW) from an accredited program.
  • Current California state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits

This position offers a competitive salary range of USD 120,000 to 130,000 annually.
