• Conduct thorough psychosocial assessments to identify clients' mental health needs, cognitive strengths, and personal resources through virtual consultations.
• Facilitate individual, family, and group therapy sessions utilizing evidence-based modalities such as cognitive-behavioral therapy (CBT) and trauma-focused therapy, tailoring interventions to fit client circumstances.
• Collaborate with other healthcare professionals, including physicians and nurse practitioners, to ensure a coordinated approach to symptom management and psychosocial support.
• Design and implement tailored treatment plans, setting realistic goals, documenting progress, and adjusting strategies as needed during ongoing sessions.
• Provide crisis intervention services and safety planning for clients experiencing acute distress or mental health crises.
• Maintain meticulous documentation and records in compliance with ethical standards, ensuring confidentiality and timely updates to care management systems.
• Lead therapeutic group sessions and educational workshops, emphasizing coping strategies and emotional processing for both patients and their families.

Your clients will primarily include individuals facing life-limiting illnesses and their families. This role requires a nuanced understanding of grief, loss, and the challenges associated with end-of-life care. You will work closely with clients from diverse backgrounds, providing empathetic support that honors their unique life experiences and personal values.

• Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in mental health or a related outpatient setting.
• Strong understanding of evidence-based treatment modalities and therapeutic techniques.
• Excellent communication and interpersonal skills, along with proficient assessment capabilities.

• Experience working in hospice, palliative care, or with terminally ill clients.
