As a Licensed Mental Health Counselor, your day-to-day activities will include:

• Assessing individual mental health needs through comprehensive patient evaluations and interviews.
• Developing, implementing, and revising personalized mental health care plans based on established standards of practice.
• Conducting one-on-one and group counseling sessions, including multiple healthy minds workshops aimed at educating patients and families.
• Effectively communicating treatment goals and plans to patients and their families, adjusting approaches as necessary based on ongoing feedback and evaluation.
• Collaborating with physicians and interdisciplinary team members to coordinate patient care and evaluations, ensuring comprehensive treatment.
• Maintaining strict confidentiality in accordance with HIPAA regulations and completing all necessary documentation in medical record systems.
• Making appropriate referrals to other healthcare professionals when necessary based on patient assessments.

You will primarily work with a diverse patient population, including those experiencing mental health challenges related to chronic conditions, particularly focusing on cardiac patients. The ideal candidate will possess:

• A valid LMHC, LCSW, LMFT, or psychologist license.
