SOUTH OF AUSTIN TERRITORY

The Home Health Field LVN/LPN plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring comfort, safety, and effective management of health conditions. This position requires the ability to assess patient health status, administer medications, and provide wound care while collaborating closely with physicians, therapists, and family members. The LVN/LPN will be responsible for developing and implementing individualized care plans that promote patient independence and improve overall well-being. This role demands strong clinical judgment and compassionate communication to educate patients and caregivers on treatment protocols and health maintenance. Ultimately, the Home Health Field LVN/LPN contributes to enhancing patient outcomes by providing personalized, attentive care in a home setting, supporting recovery and chronic disease management outside of traditional clinical environments.

As a Home Health LVN, you will:

  • Under the direction of the RN, assist in identifying the patientโ€™s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.

  • Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.

  • Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.

  • Assure the continuity of care through delivery of quality patient care.

  • Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.

  • Monitor assigned cases to ensure compliance with requirements of third party payor.

  • Promote Agency philosophy and administrative policies to ensure quality of care.

  • Current LVN License

  • Valid Driverโ€™s License

  • Valid Auto Insurance

  • CPR Certification

  • HomeCare HomeBase experience preferred
