About Us

Nest Home Hospice provides compassionate, patient-centered hospice care to individuals and families throughout the Houston area. We're looking for a dependable, caring Hospice/Home Health Aide to join our clinical team on a PRN basis.


Job Summary

The Hospice/Home Health Aide provides direct personal care and related services to patients in their place of residence, working under the direction and supervision of a Registered Nurse. This role helps maintain a safe, clean environment for patients while communicating observations and concerns to the care team to ensure continuity of care.


Responsibilities

  • Provide direct patient care under the direction of the RN and according to the Aide Plan of Care
  • Assist patients with safe transfers and ambulation per agency policy
  • Report changes in patient condition and document care provided to ensure continuity of care
  • Practice accepted infection control principles; maintain a clean, safe, and comfortable environment
  • Assist patients with proper nutrition and adequate fluid intake
  • Perform treatments and procedures according to agency policy
  • Promote positive, respectful communication with patients, families, and team members
  • Respect patient privacy, dignity, and property at all times


Qualifications

  • High school diploma preferred
  • Completion of one of the following: a state-approved Nurse Aide/Home Health Aide training and competency evaluation program (per 42 CFR §418.76), or a state licensure program meeting the same requirements
  • At least 1 year of experience as a hospice aide, home health aide, or nursing assistant preferred (hospital, nursing home, home health agency, hospice, or long-term care setting), or completion of a CNA/CHHA program
  • Must pass an Aide Competency Evaluation Skills Checklist and written skills test
  • Ability to read, write, and follow instructions in English
  • Valid driver's license, reliable transportation, and current auto liability insurance
  • Must pass a criminal history background check


Physical Requirements

  • Prolonged standing and walking
  • Ability to lift/move up to 50 lbs. and assist with patient transfers
  • Manual dexterity and hand-eye coordination for use of durable medical equipment
  • Ability to work a flexible schedule and travel locally between patient homes
  • Occasional exposure to blood, body fluids, and infectious materials; standard precautions required
