• Conduct in-depth clinical assessments to evaluate the emotional, psychological, and social needs of patients and their families.
• Create individualized treatment plans that incorporate solution-focused interventions, addressing patients' mental health challenges associated with end-of-life care.
• Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy and supportive counseling, tailored to meet the diverse needs of hospice patients.
• Maintain accurate and timely clinical records while adhering to regulatory and ethical standards.
• Collaborate with a multidisciplinary team, including nurses, physicians, and chaplains, to ensure coordinated and holistic patient care.
• Participate in case consultations and peer review meetings to continually enhance the quality of care provided.
• Educate and support patients and their families about mental health resources, coping mechanisms, and available community services.
• Stay informed on the latest research and developments in the field of social work and hospice care to inform practice and improve patient outcomes.

You will primarily work with individuals facing terminal illnesses and their families, addressing the emotional and psychological impacts that accompany these life-altering experiences. Your expertise will help empower patients to navigate their circumstances with dignity and support, ultimately enhancing their overall well-being as they transition through this critical stage of life.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Georgia.
• A minimum of three years of clinical experience specializing in psychotherapy and gerontological social work.
• Proficiency in solution-based psychotherapy interventions and familiarity with hospice care dynamics.
• Exceptional communication and interpersonal skills, with a demonstrated ability to build rapport with diverse populations.
• A collaborative approach to working with a multidisciplinary team, fostering open communication and shared decision-making.

• Experience working in hospice or palliative care settings.
• Knowledge of local community resources and services that support hospice patients and their families.
• Training in trauma-informed care and bereavement support.
