Key Responsibilities

As a Licensed Clinical Social Worker, your daily responsibilities will include:

• Conducting thorough psychosocial assessments through interviews, observational techniques, and medical record reviews to identify emotional, social, and environmental factors affecting the patient’s experience.
• Providing compassionate counseling and emotional support to patients and their families as they cope with end-of-life issues, gain clarity on treatment options, and navigate challenging care decisions.
• Collaborating with interdisciplinary teams, including nurses, physicians, and chaplains, to develop individualized treatment plans that address patients' psychosocial needs and enhance their overall well-being.
• Facilitating access to community resources such as grief support groups, counseling services, and practical assistance for families navigating end-of-life care.
• Participation in team meetings to discuss patient progress and adjustments to care plans, ensuring effective communication within the healthcare team.
• Contributing to discharge planning and follow-up services to ensure seamless transitions for patients and families seeking continued support post-hospice care.

Patient Population / Client Focus

In this role, you will work with a diverse group of patients facing terminal illnesses, including cancer, dementia, and other life-limiting conditions. Your holistic approach will ensure that you address not only the clinical care but also tackle the emotional and psychological challenges that arise during such delicate phases of life. Supporting families through grief and facilitating open conversations about preferences for end-of-life care will be paramount to your responsibilities.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• A valid and active License as a Clinical Social Worker (LCSW) in the state of Kansas.
• Proven knowledge of end-of-life issues, coping mechanisms, and the complexity of grief.
• Outstanding communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.

Preferred Experience

While not mandatory, we highly value experience in a rehabilitation or medical setting. Familiarity with hospice or palliative care frameworks will be advantageous in this role. A background in crisis intervention and a deep understanding of the social determinants of health specific to vulnerable populations will enhance your contribution to patient care.

Compensation and Benefits
