Work Environment

This remote position allows for flexibility while maintaining a structured work schedule. Our typical work hours are Monday through Friday, from 8:00 AM to 4:30 PM, with potential evening or weekend hours required to meet client needs. This role demands flexibility and might involve on-call availability to address urgent client situations. You will have the autonomy to manage your schedule, ensuring that you can provide seamless support to the seniors you serve.

Key Responsibilities

• Perform comprehensive psychosocial assessments of seniors to understand their emotional, mental, and social well-being, formulating personalized care plans based on their unique goals.
• Provide counseling and emotional support to clients and their families, facilitating discussions about life transitions, challenges, and coping mechanisms.
• Advocate for clients by assisting them in navigating community resources and ensuring they have access to necessary support services.
• Collaborate with an interdisciplinary team, including healthcare professionals, to coordinate care approaches and enhance the overall well-being of clients.
• Conduct regular follow-up visits with clients to monitor changes in their living conditions and overall health, addressing any safety concerns.
• Educate clients and their families about community resources, support groups, and programs that can enhance social engagement.
• Document all client interactions thoroughly, maintaining accurate and up-to-date records of assessments, care plans, and progress notes.
• Remain informed about relevant social work practices, policies, and ethical standards to ensure compliance and provide the highest quality of care.

Patient Population / Client Focus

You will primarily work with senior clients facing a variety of emotional and social challenges, potentially including grief, depression, anxiety, and family dynamics. Your expertise will be vital in helping these clients navigate their unique circumstances, advocating for their needs, and promoting their overall wellness and independence.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current and valid LCSW licensure in the state of Kansas.
• A minimum of two years of experience in a clinical social work role, ideally within a geriatric or community health setting.
• Strong understanding of social work principles, best practices related to aging issues, and knowledge of available community resources.
• Demonstrated ability to communicate effectively, with strong assessment, counseling, and problem-solving skills.
• Empathetic, compassionate approach with a proven ability to build rapport with seniors and their families.
