CloseKnit is an integrated hybrid primary care practice expanding into the Massachusetts market. CloseKnit serves commercial and public insurance populations. We are seeking a compassionate and experienced Remote Mental Health Therapist to provide high-quality therapeutic services through a virtual platform to our patients. The therapist will support clients in developing coping skills, improving emotional well-being, and achieving personal goals while collaborating with families and interdisciplinary care teams to deliver coordinated, patient-centered care.
Requirements
Key Responsibilities
Assessment & Evaluation:
  • Conduct comprehensive assessments of behavioral, emotional, and mental health needs.
  • Develop individualized treatment plans aligned with client goals.

Therapeutic Interventions:
  • Provide evidence-based individual, family, and group therapy using modalities such as CBT and DBT.
  • Deliver care both onsite and virtually in alignment with the hybrid care model.

Monitoring & Documentation:
  • Track client progress and adjust treatment plans as needed.
  • Maintain accurate and timely clinical documentation, including progress notes and treatment outcomes.

Collaboration & Communication:
  • Work collaboratively with Primary care physicians, psychiatrists, social workers, case managers, and other healthcare professionals.
  • Communicate treatment plans, progress, and recommendations with clients, families, and stakeholders.

Crisis Intervention:
  • Provide crisis intervention and support to clients in acute distress.
  • Develop safety plans and strategies to prevent or mitigate crises.

Education & Support:
  • Educate clients and families on mental health conditions, treatment options, and coping strategies.

Caseload Expectations:
  • Maintain a minimum caseload of 25+ clinical sessions per week.
  • Full-time 40 hours per week. Flexible scheduling options to include evenings and weekends.

Required Qualifications:
  • Master's degree in social work, Counseling, or related behavioral health field.
  • Active licensure (must be able to practice independently) in at least one of the following states: Massachusetts (MA), Washington, DC, Maryland (MD), Virginia (VA).
  • 3-5 years of clinical experience working with youth, adults, and individuals with behavioral health challenges.
  • Experience working with patients throughout the lifespan (ages 7 and above)
  • Strong skills in evidence-based therapeutic practices.
  • Excellent communication, interpersonal, and organizational abilities.

Preferred Qualifications:
  • Experience with specific treatment populations or specialties (e.g., trauma, substance use).
  • Proficiency with Athena EMR.
  • Familiarity with EHR documentation standards.
  • Bilingual is a plus but not required.
  • Remote experience is highly preferred.
