Key Responsibilities

  • Initiate follow-up checks to gauge patient progress and adjust treatment approaches as necessary.
  • Develop relapse prevention plans to help patients maintain stable functioning, assisting them in managing situations that could lead to further mental health challenges.
  • Monitor and coordinate behavioral health services, linking with other treatment providers as appropriate and with patient consent.
  • Maintain accurate documentation of patient progress and diagnostic information in treatment charts.
  • Keep primary care providers informed about patients' needs and progress, collaborating to formulate comprehensive treatment plans.
  • Facilitate behavioral changes in patients at risk for physical disorders, promoting healthier lifestyle choices.
  • Equip patients with self-management skills and educational resources, empowering them to actively participate in their treatment and recovery.
  • Consult with clinic management and team members regarding behavioral services and program evaluation.
  • Support patients in complying with medical treatments initiated by their primary care providers, offering strategies for handling medication side effects.
  • Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while fulfilling additional duties as assigned.
