This full-time position operates in a remote work environment, providing flexibility to promote work-life balance while ensuring that our clients receive consistent and accessible care. As a member of a community health clinic, you will collaborate with various interdisciplinary teams to implement individualized care plans tailored to each client's needs.

Key Responsibilities
  • Conduct comprehensive assessments and create detailed treatment plans that include pre-crisis and crisis strategies, behavioral guidelines, risk assessments, and medication treatment plans.
  • Document client interactions, assessments, and treatment progress in a timely manner, adhering to all regulatory requirements and organizational standards.
  • Utilize evidence-based screening tools as part of the clinical assessment process, ensuring that the treatments provided are backed by current best practices.
  • Collaborate with interdisciplinary teams to create and implement positive behavior support plans to enhance client outcomes.
  • Engage in all processes related to licensing and accreditation, ensuring that the clinic meets the highest standards of care.
  • Serve as a liaison between clients, appropriate service systems, state agencies, and community partners to facilitate comprehensive care.
  • Establish and maintain effective communication strategies to uphold contract standards and promote our mission within the community.
  • Foster relationships with local, state, federal, and private organizations to promote services and cultivate potential partnerships and funding sources.
  • Commit to personal and professional development by attending workshops, conferences, and participating in relevant committees and task forces.
  • Promote the agency’s core mission, values, and principles in all interactions and functions.
  • Complete any other duties as assigned by the Clinic Director to support operational needs and enhance service delivery.
Patient Population / Client Focus

Qualifications
