Key Responsibilities
  • Conduct comprehensive assessments and ongoing evaluations to tailor treatments according to the unique needs of each patient.
  • Facilitate effective communication between various stakeholders, including supervisors, directly involved care staff, family members, and guardians, regarding patient progress and treatment plans.
  • Develop and maintain tentative schedules for appointments, home visits, and observations to monitor individuals' advancements and provide necessary interventions.
  • Coordinate home visits that involve in-home observations, chart reviews, and staff interviews, ensuring a holistic understanding of patient circumstances.
  • Implement treatment plans and ensure they are executed effectively, while actively participating in interdisciplinary team meetings to discuss patient progress and strategies.
  • Document monthly and quarterly progress notes that reflect patient growth and areas needing further attention, using EMR software to maintain systematized records.
  • Organize training sessions for staff to guarantee competency in executing treatment plans and documentation requirements.
  • Provide ongoing support through follow-up interviews and observations to ensure patients are progressing in their treatment.
  • Act as a liaison with community resources and mental health organizations to enhance support for clients' development and recovery goals.
  • Contribute to the creation of procedures and materials that enhance the team's training and program delivery.
Patient Population / Client Focus

Your role will involve working with a diverse client population, including both adults and youth facing challenges such as anxiety, depression, trauma, and substance misuse. This variety in client backgrounds will require a tailored approach to counseling, allowing you to make impactful differences in the lives of individuals from various walks of life.

Qualifications
  • Master's or Doctoral degree in Counseling, Psychology, or a closely related field.
  • Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee.
  • Minimum of two years of counseling experience in a clinical setting is mandatory.
  • Demonstrated understanding of legal and ethical standards governing mental health practice.
  • Exceptional interpersonal and communication skills, with the ability to build rapport with a wide range of clients.
Preferred Experience
  • Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance abuse counseling is highly desirable.
  • Proficiency with Electronic Medical Records (EMR) software and related technologies.
  • A collaborative spirit with a proven ability to work effectively within a multidisciplinary healthcare team.
Compensation and Benefits

The salary for this position ranges from $90,000 to $100,000 per year, commensurate with experience and caseload.
