Remote Health Risk Assessment Nurse Practitioner information

What does a remote health risk assessment nurse practitioner do?

A Remote Health Risk Assessment Nurse Practitioner evaluates patients’ health status by conducting comprehensive assessments, typically via phone or video calls. They review medical histories, current conditions, and risk factors to identify care gaps and recommend preventive measures. The goal is to support improved health outcomes, often for insurance companies or telehealth providers, without providing direct treatment. This role enables flexible work-from-home opportunities while still making a meaningful impact on patient care.

What are the key skills and qualifications needed to thrive as a remote health risk assessment nurse practitioner, and why are they important?

To thrive as a Remote Health Risk Assessment Nurse Practitioner, you need advanced clinical assessment skills, a graduate nursing degree, and an active NP license, often with experience in geriatrics or chronic care. Familiarity with telehealth platforms, electronic health records (EHRs), and risk assessment tools is essential. Outstanding communication, self-motivation, and attention to detail are crucial soft skills for building rapport and ensuring accurate remote evaluations. These abilities are vital for delivering quality care, accurately identifying health risks, and supporting patient outcomes in a virtual setting.

What are some typical challenges a remote health risk assessment nurse practitioner might face, and how can they be managed?

Remote Health Risk Assessment Nurse Practitioners often encounter challenges such as limited in-person patient interaction, technological hurdles, and managing a high volume of assessments. To overcome these, it's important to develop strong communication skills for virtual settings, familiarize yourself with telehealth platforms, and establish an organized workflow. Staying connected with your remote team for support and ongoing training can also help ensure you provide accurate and compassionate care while adapting to the remote work environment.

Californiaapplicants must hold FILED 103/104 independent practice authority AND activeMedi-Cal (fee-for-service) enrollment — please do not apply without both.

What'sin it for you:

  • Flexible Part-Time Schedule: W-2 role — as little as 10 hours per week in consistent weekly blocks that you set about a month in advance. Weekdays only — no nights, no weekends.
  • Competitive Pay: $85 per completed, QA-approved visit — every visit type. A typical full day runs 6–8 visits ($510–680+).
  • Fully Remote: 100% work from home. An in-home care team handles vitals, labs, and diagnostics in the patient's home while you complete the clinical assessment virtually.

  • High, Steady Volume: California is our highest-demand market. Visits are matched to you in real time through alive queue — no pre-booked appointments, which means you won’t experiencepatient no-shows.

Requirements:

  • Licensure: Active, unencumbered California NP licensure with filed 103/104 independent practice authority and active Medi-Cal (FFS) enrollment. California-only licensure is welcome — additional state licenses a plus.
  • Experience: 1+ year as a practicing Nurse Practitioner. Annual Wellness Visit, Health Risk Assessment, or telehealth experience strongly preferred.
  • Board Certification: FNP, AGNP, Adult, or Acute Care.
  • Availability: Minimum 10 hours per week, Monday–Friday, 7:30am–4:00pm Pacific. Two full 8-hour days per week is ideal.
